FINANCIAL POLICY AND INSURANCE STATEMENT

We are committed to providing you with the best care possible. In order to achieve these goals, we need your assistance and your understanding of our payment policy. Payment for services is due at the time services are rendered. We accept cash, checks, MasterCard and VISA. We will provide guidance regarding lending sources, if necessary.

Ours is a specialist office and even if you have been referred by your dentist, you may receive medical/surgical care. We are permitted to seek payment for non-covered services and we are required to inform you of these services prior to performing treatment. Our customary procedure in handling patient accounts is the same for patients with or without insurance coverage. An estimate of the fee is given. We then require definite financial arrangements be made before treatment is performed.

ur office is happy to cooperate with those who have health insurance. We only ask that you read your policy to be sure that you are fully aware of any limitations of the benefits provided. We are required to collect deductible as well as co-payment amounts. We will bill your insurance (including Medicare) for the benefits due to you. If for any reason payment does not represent the entire amount due, we are authorized to collect the balance from you.

Your policy may base its allowances on a fixed fee schedule which may or may not coincide with our fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Some companies pay claims promptly, while others delay payments many months. Since we have no say in the selection of your insurance company, we ask that you look upon your insurance policy realistically as a means that reimburses you for health care expenses. It is your policy, and it is your responsibility to see that reimbursement is prompt. As a courtesy service to you we will complete all forms pertaining to your claim and submit them promptly. This often involves detailed paperwork, but we are willing to do it because we realize how important it is to you.

The fees we charge for services rendered to those who are insured are usual, customary and reasonable (U.C.R.) for this region and are charged to all patients for similar services. Our fees are based on the complexity of the procedure(s), the high level of skill required, the caliber of the office personnel, the facilities, equipment and continued education mandated to comply with standards of care, as well as the time involved in performing quality and compassionate care.
